They will select your name from the department drop down. The notification will come to you from Formstack at [email protected] . First time Formstack users must be added prior to gaining access.  Please let [email protected]  know if you and/ or your department need to be added to the dropdown list.

After you fill out the department, faculty sponsor, and program chair sections of the application, it will come to the Office of the University Registrar (OUR)  for processing. Once the complete application reaches OUR, it takes roughly 3 days to be processed and for the visitor to gain Health Science Center credentials. Once credentials are created, OUR will email the visitor and the department with the information. Memos are not required.

The fee is $25 per academic cycle (7/1 – 6/30) and is non-refundable and non-transferable. The fee must be paid when completing the electronic application.

If the department is paying, an IDT can be submitted through Team Dynamix. OUR does not send a bill for the visitor. The Team Dynamix IDT must be initiated by the department. Information for the Team Dynamix is below account.
