Change of CIP Code

The program submits to the Vice President for Academic, Faculty and Student Affairs the following documents:

1. Justification Memo

2. THECB CIP Code Change Request Form

Vice President for Academic, Faculty and Student Affairs submits the CIP Code Change Request form to UT System Office of Health Affairs

UT System submits CIP Code Change Request form to THECB via portal

THECB review prior to approval

UT System approves the change
