Change of Departmental Name Change

School Curriculum Committee approves of department name change.

Program Director/Dean submits to Vice President for Academic, Faculty and Student Affairs submits the following:

1. Justification memo of intended academic administrative changes

2. Program completes THECB Notification of Administrative Changes form

3. 30 day notification and comment period to area IHE's whini a 50 mile radius

Any objections from area IHE's

Vice President for Academic, Faculty and Student Affairs submits form to UT System Executive Vice Chancellor for Health Affairs

UT System Executive Vice Chancellor for Health Affairs reviews and submits the form to THECB via the portal

THECB reviews and approval granted
