Program Director/Dean submits to Vice President for Academic, Faculty and Student Affairs submits the following:

Any objections from area IHE's

Continue on

Stop the process and revisit the change

Vice President for Academic, Faculty and Student Affairs submits form to UT System Executive Vice Chancellor for Health Affairs

UT System Executive Vice Chancellor for Health Affairs reviews and submits the form to THECB via the portal

THECB reviews and approval granted

After approval, UT Health submits courtesy notification to SACSCOC
